RELEASE AND CONSENT

1. I, the undersigned, hereby grant full permission to Creditrepair.com, Inc., its parent, subsidiary, and affiliate entities (collectively "Producer"), any of its subsequent assignees, and anyone authorized by Producer, the right to photograph me and/or to record performances, poses, actions, plays, voice, and appearances, and to use my name, likeness, biographical information, testimonial, picture, photograph, silhouette and other reproductions of my physical likeness (the "Likenesses") in all broadcast or print mediums including, but not limited to, television, radio, and Internet to educate, influence, and promote the services of Producer and any of its vendors and/or clients.  I hereby release Producer from all liability in connection with the Likenesses.

2. I hereby grant to Producer, its successors, assigns and licensees the perpetual right to use, as it may desire, the Likenesses.

3. I understand that I will not receive payment for such use of my Likenesses.  However, I may be entitled to other compensation.

4. I hereby grant Producer final approval over the use of my Likenesses.

5. I have read every word of this document, or have had all of it read to me, before I signed it, and I fully understand its legal effect.  I hereby certify and represent that I am over 18 years of age.  I warrant that by signing this document, I am not violating any other commitment.
